Duties consist of but are not limited to the following:
This position requires travel, flexible hours and must be available to work weekends and nights
Provides moderately complex case management work.
Performs clinical screenings, assessments, and service eligibility evaluations based on consumer needs.
Coordinates and links appropriate services and monitors consumer progress with meeting treatment goals.
Delivers interventions such as crisis resolution, case management services and discharge planning.
Provides clinical services to address the need for ongoing services and/or inpatient admission.
Provides crisis follow-up and relapse prevention services when appropriate.

Qualifications
Graduation from an accredited four-year college or university with a major course work in a related program to this area required such as Human/Social Services or Psychology.
Preferably have a least three years experience working with mental health consumers in a crisis situation and also experience working with consumers in the home/community environment.
Must have basic understanding of substance abuse and related issues.
Valid Texas driverβs license, a good driving record for the past three years and a valid auto insurance card. Must be familiar with and/or willing to be training in trauma informed care.
This includes understanding, recognizing, and responding to effects of all types of trauma.
Must be able to utilize electronic health record system to document collaboratively and concurrently with consumers across all processes including assessment, service planning, and ongoing consumer - provider interactions.
Must have the ability to work in a behavioral environment where staff convey dignity, respect, hopefulness, and the opportunity for choice and empowerment among consumers.
PHYSICAL ABILITIES: Must be able to bend, stoop, kneel, crouch, squat, lift at least 25 pounds, and reach overhead
